PROGRAM DETAILS

The pharmacy fills prescriptions for eligible patients seen at GHCCM’s Medical Clinic, Kintegra Family Medicine, CVMC Claremont, Catawba Valley Health, Cognitive Connections, Catawba Valley Medical Group practices and many others. In addition, patients seen in the emergency departments, urgent cares, or other primary care or specialty practices can have prescriptions filled at the GHCCM pharmacy if they are eligible. The Pharmacy has 2 lists of medications for qualifying CCM patients. Tier 1 medications are donated and are dispensed at zero cost to the patient. Tier 2 medications require patients to pay a $4.00 or higher administrative fee. Differences in these administrative fees may change due to fluctuation in acquisition cost.

ELIGIBILITY

  • Individuals must be a current patient of Kintegra Family Medicine in Hickory or Catawba Valley Family Medicine in Claremont or an associate thereof, have a current GHCCM Healthcare Card, or be a current employee or household family member of GHCCM.
  • GHCCM Healthcare Card eligibility requirements: See our Client Services department or call (828) 327-0979 EXT 221
